2022-2023 Travel Team Registration

for SQ/U10 - PW/U12 - B/U15

1. Obtain Player USA Hockey Confirmation Number

2. Complete on-line registration
   • This link is for Squirts and OLDER only! Your player should be at minimum 9 years old as of 5/31/13 to register here...

3. Make sure you have completed the Volunteer Commitment / Donation Form and filled out a Volunteer Commitment / Donation check for registration night. (Must be a separate check from Registration fees.)

4. Come to the Four Seasons Center with player and shoulder pad/chest protector on August 3rd from 5-8 pm to get sized for jerseys (if new to OYHA), pick up this year's practice jersey and raffle tickets, and drop off the following REQUIRED registration information:
   • Volunteer Commitment / Donation form & separate check
   • Birth Certificate (Required for all players new to OYHA - unless scanned and emailed to oyharegistrar@gmail.com)
   •  All other paper forms and checks as noted in the registration.

***Your options for registration payment include check payments and/or credit card payments.

***If your player is considering trying out to play hockey for OHS, the player SHOULD still go through registration with OYHA on August 3rd, and complete payment. If the player tries out and makes a high school team, parents will be reimbursed for the remaining portion of the registration fees.

• Team Managers and anyone wanting to be a locker room parent this season MUST:
  1. Obtain Parent USA Hockey Numbers
     • Click Here to get your USA Hockey Numbers
       • Select Register Now
       • Ice Manager/Volunteer for Parent Number (Free)
          
  2.  Use Parent USA Hockey # to complete background screening if a screening wasn't completed in the 2020-2021 season. Background screenings must be done every other year.

  3. Complete SafeSport training or refresher (no cost) . This is now done each year. Please allow 2 hours to complete training for the first time.
